Minimum Requirements

  • Licensure / Certification / Registration: Certified BLS Provider specializing in Health Service Provider credentialed from the American Heart Association (AHA) obtained prior to hire date or job transfer date. Or military CPR-First Aid training required.
  • Licensed Registered Nurse credentialed from the Maryland Board of Nursing or current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date required.

Additional Preferences

  • Case Management Expertise: A minimum of 3 years of dedicated experience in a Case Management role within a healthcare setting.
  • Discharge Planning Expertise: Demonstrated experience in coordinating complex patient transitions and multi-disciplinary care plans.

What You’ll Make an Impact

Coordinate comprehensive care transitions by serving as the primary link between all clinical disciplines, ensuring every patient receives a seamless and well-integrated recovery experience.

Design individualized discharge pathways that integrate patient values and family preferences, ensuring a safe and sustainable transition from the hospital to the next stage of care.

Advocate for vital patient resources by identifying and removing systemic barriers to care, ensuring every individual has the medical necessity and support required for their specific diagnosis.

Evaluate clinical documentation accuracy to provide expert recommendations on levels of care, ensuring that medical records reflect the true complexity of the patient's journey and treatment options.

Drive continuous care improvement through active collaboration with the care transition team, constantly re-evaluating health plans to adapt to a patient’s changing condition in real-time.

Pay

$50.00 per hour

Schedule

Day-Shift | PRN | 8:00AM-4:30PM, weekday and weekend availability.
