About the role
The HUD-VASH Registered Nurse Care Manager supports the HUD-VASH program by providing medical and mental health consultative services to veterans facing housing challenges.
Responsibilities
- Consults with the Lead HUD-VASH case manager and other relevant parties to address the treatment needs of veterans.
- Delivers fundamental knowledge-based care to assigned clients while developing technical competencies.
- Coordinates and monitors outpatient treatment methods in consultation with other healthcare providers.
- Provides comprehensive nursing care, including psychosocial assessments, treatment implementation, referrals, and case management.
- Makes home visits to veterans with physical and mental health issues to help them maintain stable housing.
- Conducts patient and family health education focusing on self-management, prevention, and wellness.
- Collaborates with other high-risk outpatient clinics for standardized care management.
Requirements
- Basic English language proficiency.
- Graduate of a nursing program approved by an accrediting agency.
- Current full, active, and unrestricted registration as a professional nurse.
- Experience in direct patient care positions.
Qualifications
- Associate Degree (ADN) or Diploma in Nursing.
- Bachelor of Science in Nursing (BSN).
- Master's Degree in Nursing (MSN).
- Doctoral degree in Nursing.
- Master's degree in a related field with a BSN.
