About the role
The Care at Home Registered Nurse is an integral part of a collaborative, multidisciplinary team supporting clients in the Care at Home Program. The program provides temporary healthcare services to individuals discharging from acute care facilities.
Responsibilities
- Completing accurate assessments of discharging needs, coordinating medications and appointments, and documenting accordingly.
- Reporting to appropriate personnel as needed and collaborating with Advanced Practice Providers, primary care providers, specialists, and community providers.
- Medication review against lists from discharging providers or facilities.
- Closely collaborating with local hospitals, skilled nursing facilities, community clinics, and other programs to review incoming referrals according to inclusion criteria.
- Accompanying Advanced Practice Providers for home visits and completing intake assessments in the Electronic Health Record (EHR).
- Delegating appropriate tasks to Medical Assistants and Community Health Workers.
- Providing direct nursing care and education to clients in their homes, ensuring all needs are met in accordance with established policies and procedures.
- Completing daily/weekly quality assurance checklists, including delegating tasks as appropriate (e.g., McKesson, CLIA).
- Affirming adherence to infection control standards.
- Participating in continuous quality improvement projects to enhance service quality and meet regulatory and accreditation standards.
- Triage all incoming referrals for the Care at Home Program and ensure admittance based on defined criteria.
- Ensuring compliance with regulatory and accreditation standards, Central Health Standard Operating Procedures, Policies, and Procedures.
Requirements
- Education: Associate's Degree from an accredited school of nursing.
- Experience: Three (3) years clinical experience in ambulatory care settings, hospitals, home health, or community agencies.
- Background: Three (3) years of experience managing populations with complex medical needs.
- Licenses and Certifications: Unrestricted Registered Nurse (RN) license in the State of Texas, Basic Life Support (BLS) certification.
