Registered Nurse / Behavioral Health, Home Health PRN

Location: Center Valley, PA (office location) – coverage area: Carbon and Monroe Counties, PA

This is a PRN, per-visit position.

Pay

$40 – $46 per hour (offer based on years of experience)

Responsibilities

• Assess nursing needs, provide direct care, and manage the patient’s care plan.
• Conduct initial and reassessment visits using observation and assessment skills; inform the physician of results and recommended interventions.
• Communicate changes in the patient’s condition and needs to the physician and other personnel.
• Document and submit assessment, progress, and discharge notes and required paperwork in a timely manner.
• Manage medical illness and perform psycho-biological interventions.
• Evaluate medications for ineffective therapy, adverse reactions, side effects, allergies, and contraindications.
• Instruct patients on ordered medication regimens and review medication compliance.
• Administer IM medications.
• Manage situational or other crises.

Requirements

• Current State RN license.
• One of the following qualifications:
  • Diploma or AA degree in nursing with at least 2 years of recent nursing experience (recommended within the last 3 years) in a psychiatric or mental health home care program, acute psychiatric or mental health hospital, or outpatient program.
  • Baccalaureate in nursing with at least 1 year of recent nursing experience (recommended within the last 3 years) in a psychiatric or mental health home care program, acute psychiatric or mental health hospital, or outpatient program.
  • Master’s degree in psychiatric or mental health nursing with at least 1 year of recent nursing experience (recommended within the last 3 years) in a psychiatric or mental health home care program, acute psychiatric or mental health hospital, or outpatient program.
• For agencies providing psychiatric services under the Medicare Home Health Benefit, the Behavioral Health/Psychiatric Home Care Nurse’s resume must be submitted to and approved by Palmetto.
• Current CPR certification.
• Licensed driver with current auto insurance, in accordance with state and/or organizational requirements, and a vehicle in good working order.
