As a registered nurse with our Hospice program, you will have the opportunity to provide compassionate and high-quality care to individuals with terminal illnesses, while also supporting their loved ones.

Responsibilities

• Assess patients' physical and emotional needs, develop and implement individualized care plans, and provide skilled nursing care in accordance with established standards and regulations
• Collaborate with the interdisciplinary team to ensure comprehensive care, including physicians, social workers, home health aides, and spiritual care providers
• Provide emotional support and bereavement counseling to patients and their families
• Document patient care and interactions accurately and thoroughly in accordance with established policies and procedures
• Participate in quality improvement initiatives and utilize data to drive improvements in patient care

Pay

In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $43.00 to USD $58.00. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions.
