Responsibilities
- Monitor resident health and wellness through routine assessments, observations, and ongoing evaluation of individual needs
- Deliver professional nursing services in alignment with individualized support plans, community standards, and regulatory requirements
- Coordinate resident transitions, including admissions, room changes, and discharges, while helping ensure a positive experience
- Maintain accurate and current records related to resident services, medication administration, and clinical observations
- Participate in care conferences, assessments, and the development and review of individualized resident plans
- Partner with residents, families, nursing staff, and interdisciplinary team members to support resident satisfaction, comfort, and overall quality of life
Requirements
- Current and active RN license in New Jersey
- CPR certification
- 1+ years of nursing experience required, preferably in long-term care setting
- Strong interpersonal and communication skills
