Position Summary
We are seeking a compassionate and experienced Registered Nurse (RN) to provide high-quality home health services.
Key Responsibilities
- Conduct initial and comprehensive nursing assessments, including OASIS documentation and medication reconciliation.
- Develop, implement, and revise individualized care plans in collaboration with physicians and the healthcare team.
- Provide skilled nursing care in accordance with physician orders and agency policies.
- Supervise Home Health Aides and LPN/LVNs, including required supervisory visits.
- Career coordination with clinicians, physicians, patients, and family members.
- Maintain accurate and timely documentation in compliance with agency and payer requirements.
- Participate in on-call, weekend rotations, and potentially observed holidays as needed.
- Support quality improvement initiatives focused on patient safety and reducing hospitalizations.
- Assist with staff orientation, training, and mentorship when applicable.
Required Qualifications
- Active, unencumbered Washington State RN License or Multi-State RN License.
- Current BLS Certification.
- Strong clinical assessment and care coordination skills.
- Ability to work independently and manage a patient caseload effectively.
Preferred Qualifications
- Home Health nursing experience.
- Experience with HCHB/PointCare documentation systems.
- Excellent communication, organization, and time-management skills.
