Registered Nurse

CASES Bronx Nathaniel Assertive Community Treatment (ACT) Team is a multi-disciplinary, alternative-to-incarceration program that provides evidence-based, mobile, community mental-health treatment to individuals 18+ with serious mental illnesses, co-occurring substance-use disorders, high emergency-service utilization, and criminal-legal involvement in Bronx courts.

About the role

The Registered Nurse delivers medication management, health assessment, counseling, and psychoeducation to promote recovery, wellness, and optimal health. At least 80% of services are provided in the community, adhering to ACT principles, professional nursing ethics, and regulatory guidelines.

Responsibilities

• Provide at least 80% of medication management and health services in the community, including:
  • Medication administration (oral and intramuscular)
  • Monitoring medication side effects and adherence
  • Phlebotomy
  • Coordination of consumers’ health-care services
  • Psychoeducation about health and medications
  • Mental-status assessments
  • Rotating on the team’s after-hours emergency line
• Complete and sign progress notes, nursing assessments, incident reports, and other documentation in compliance with New York State Office of Mental Health (OMH), NYC Department of Health and Mental Hygiene (DOHMH), and Medicaid billing regulations.
• Collaborate with prescribers and Licensed Practical Nurses (LPNs) on consumers’ medication needs, health-care services, and medication-administration schedules.
• Provide direction to LPNs regarding medication management, health education, counseling, and psychoeducation.
• Coordinate care with family, community providers, hospitals, housing providers, shelter staff, criminal-justice personnel, Assisted Outpatient Treatment (AOT), and primary-care providers.
• Participate in daily team meetings, clinical meetings, and monthly nurses meetings to discuss clinical issues and client needs.
• Use motivational interviewing, culturally and developmentally appropriate, recovery-oriented, and trauma-informed approaches.
• Perform other duties as required by the Team Leader and senior clinical leadership.

Requirements

• Current New York State Registered Nurse (RN) license.
• Minimum of three years’ experience working with adults (18+) in one or more of the following:
  • Psychiatric nursing
  • Substance-use treatment settings
  • Social-service settings serving individuals experiencing homelessness, criminal-legal involvement, or limited social support
• Extensive knowledge of community treatment, support services, and resources.
• Highly organized and self-motivated.
• Proficient in computer use.
• Able to conduct physical outreach visits in the community at least 80% of the time.

Preferred Skills

• Fluency in Spanish.

Pay

$103,968 per year.

Schedule

• Full-time, Monday–Friday, 9:00 AM–5:00 PM.
• 35 hours per week excluding breaks.
• Flexible work arrangements: staff may complete their workday from home or another suitable space after reporting to the office and/or engaging in field or community-based work.
