About the role

We are seeking a Registered Nurse to join our Care Coordination/Case Management Team. This full-time position could be based in Duluth, Hibbing, or Virginia, MN based on the preference of the selected candidate.

Responsibilities

• Coordinate care and services to clients age 65+ that are enrolled with Blue Cross and Blue Shield managed care plans.
• Complete comprehensive health risk assessments at least annually for health and social needs.
• Develop, review, and maintain individualized, comprehensive care plans for clients.
• Serve as the client's advocate.

Requirements

• Graduation from an accredited college or university with an Associate’s degree in nursing.
• Possession of a current Minnesota Registered Nurse license.
• Possession of a valid driver’s license.
• Two (2) years full-time paid verifiable experience in a home or community-based setting.
• Possession of MnCHOICES Certified Assessors certification within two (2) months of employment. Failure to obtain the certification within this period will result in termination.
