Position Overview

We are seeking a compassionate and dedicated Registered Nurse (RN) to join our team. The RN will assess, plan, and oversee personalized care plans for clients, ensuring high-quality in-home care.

Key Responsibilities

• Conduct comprehensive evaluations of clients' physical, mental, and psychosocial needs to develop individualized care plans.
• Collaborate with clients, families, and the care team to create and update personalized care plans addressing identified needs.
• Monitor and coordinate services to ensure adherence to care plans, making adjustments as necessary to meet changing client needs.
• Provide guidance and mentorship to caregivers, ensuring they are well-informed about care plans and best practices.
• Maintain accurate and timely records of assessments, care plans, and any changes in client conditions.
• Ensure all care practices comply with federal, state, and local regulations, as well as company policies.

Qualifications

• Active Registered Nurse (RN) license in the state of South Carolina.
• Bachelor of Science in Nursing (BSN) preferred.
• Minimum of one year of experience in long-term care or home health care settings.
• Strong assessment, critical thinking, and problem-solving skills.
• Excellent communication and interpersonal abilities, with a compassionate approach to client care.
• Ability to work independently and as part of a collaborative team.
• Proficiency in electronic health records (EHR) and other relevant software.
