What You Will Do - Essential Functions The registered nurse will be based in the FL market. With responsibility for supporting the patient panel attributed to their care team pod, in partnership with their Advanced Practice Provider and Community Health Worker this individual will have specific responsibility for both direct care delivery and care coordination. Care Coordination & Clinical SupportCoordinate patient-centered care by partnering with providers, care teams, specialists, community resources, and other clinical partners to support seamless care delivery and transitions. Conduct patient outreach, monitor health status, provide education and medication support, address preventive care needs, and escalate clinical concerns to providers as appropriate.Care Planning & CommunicationCollaborate with providers and care team members through huddles, case discussions, and care planning activities to address patient needs and goals. Maintain clear communication with patients, caregivers, and care partners, ensuring care plan updates, instructions, and follow-up needs are effectively shared.Scheduling & DocumentationManage provider schedules and coordinate routine, urgent, and post-hospital visits to support timely patient access and continuity of care. Maintain accurate and timely documentation of patient encounters, care coordination activities, and clinical interventions to support compliance, quality outcomes, and reporting. Who You Are Required Qualifications: Active, unrestricted Registered Nurse (RN) license in the state of Florida.Associate’s Degree in Nursing (ADN) or higher.2+ years of clinical nursing experience, preferably in primary care, care management, internal medicine, home health, or a closely related setting.Demonstrated ability to perform clinical triage, assess acute changes in condition, and manage chronic conditions.Strong care coordination, communication, and patient education skills.Ability to work effectively within an interdisciplinary care team.Proficiency with electronic health records (EHRs) and clinical documentation standards.Bilingual preferredFor candidates based in Florida, this role includes facilitating in-person meetings and supporting face-to-face member interactions.
