Roles and Responsibilities
- Complete initial nursing assessments and Discharge summaries for DD Waiver and ICF-IID
- Create and submit HCP, MERPS, CARMP, annually, following State Standards
- Initiate and complete Nursing portion of CARMP
- Train and delegate HCC on all Individual's HCP / MERPs and CARMPs
- Review and sign all appointment consultation forms. Communicate all follow ups to HCC
- Monthly Review and Oversight of MARS: Review, Update, and communicate with HCC all follow-ups with physicians' offices, and pharmacy. Complete Medication Review Form.
- Complete and update Therap Documentation, as appropriate, for Individuals receiving services, including, but not limited to: E-CHAT/E-CHAT Summary; MAAT; ARST; current ICD/DSM codes for medications and diagnoses.
- Review in Therap and communicate to HCC ALL: Medication Changes, Appointments Needs, Health Tracking and Vitals Needed
- Medical concerns documented in T-logs or ISP Data
- Provide follow-up regarding GERs that involve Individual's health and medical issues, including the use of ER, urgent care, and/or EMT services; injuries; medication errors.
- Attend Individual's / Great Livin meetings as needed, to address health and medical needs, including, but not limited to:
- ISP/IDT meetings
- Management Team meetings
- Basic Assurances
- HRC
- Dr. appointments, audits, etc.
- Correspond with Teams, medical providers and/or other agencies to address Individual's health and medical issues/concerns.
- Participate in Pharmacy reviews for services provided by Great Livin as needed
- Implement training and oversight of CMA's through GL's CMA program and supervise after receiving CMA certificate.
Knowledge and Skills
- Must be able to speak, read and write in English.
- Profound written skills and should have proficiency in handling computer programs used in Great Livin's health services management.
- Knowledge of agency positions and responsibilities, and who to go to with specific questions and, or concerns.
- Must be of good integrity and possess adequate physical, mental, and emotional stability so as to meet job duties in a safe and responsible manner.
- A general understanding of the New Mexico ICF-IDD Standards and a working knowledge of Board of Nursing regulations and requirements as they pertain to Great Livin's CMA education program and CMA services.
- Ability to establish priorities, work independently, good leadership skills, and proceed with objectives without hands-on supervision.
- Ability to handle and resolve recurring problems.
- Willing to work the hours necessary to meet the needs of the individuals with whom you work.

Special Requirements
- Valid New Mexico Driver's License.
- Valid Auto Liability Insurance.
- Professional Liability Insurance with limits of $1,000,000 (each claim) and $3,000,000 aggregate.
- Pass required background checks.
- New Mexico Professional License
Qualifications
- Graduated from an accredited nursing program and meet the requirements outlined by the State of New Mexico.
- Has obtained a valid Nursing License in New Mexico.
