Registered Nurse
About the role
The Registered Nurse will work with the CN Guidance's Mobile Residential Support Team and all Community Residence Programs to deliver services to individuals diagnosed with mental illness living in various levels of housing. The role involves providing direct patient care, implementing treatment plans, and coordinating client services.
Responsibilities
- Coordinate client services including scheduling appointments, obtaining UA, and administering IM and subcutaneous injections.
- Conduct medication reconciliation and vitals checks.
- Ensure completion of medical assessments and coordinate additional services with medical providers.
- Monitor and report on the effectiveness of prescribed medications.
- Facilitate patient education on medication benefits, risks, and side effects.
- Schedule clients for visits and notify medical providers of medication renewal needs.
- Provide 24/7 coverage as needed.
- Enter documentation into the electronic health record within 24-72 hours post-visit.
- Facilitate data sharing with clients for informed decision-making.
- Participate in Quality Improvement (QI) training and activities.
- Support a culture of inclusion and adhere to safety protocols.
Requirements
- Current New York State Registered Nurse license.
- Associate's or Bachelor's degree in Nursing, with a Master's degree preferred.
- At least one year of community mental health experience and/or two years of mental health experience in a setting.
Qualifications
Qualified candidates should have a strong background in mental health nursing and experience working with individuals diagnosed with mental illness. Knowledge of health and safety protocols is essential.
Skills
Strong communication skills, ability to manage multiple tasks, and proficiency in electronic health records are required. Experience with mental health interventions and crisis management is beneficial.

Pay
$46.15 - $49.74 per hour
Schedule
Full-time position requiring 24/7 coverage.
