About the role
Baptist Health Home Health/Hospice RN Case Manager provides, manages and coordinates care and services for an assigned caseload of patients.
Responsibilities
- Provides, manages and coordinates care and services for an assigned caseload of patients.
- Manages patient care plans and ensures compliance with regulatory standards.
- Communicates with physicians, other healthcare providers, and patients to ensure coordinated care.
- Assesses patient needs and develops appropriate care plans.
- Coordinates referrals to other healthcare professionals and resources as needed.
- Ensures timely and accurate documentation of patient care activities.
- Maintains confidentiality of patient information.
Requirements
- Education required: Graduation from an approved program of Nursing.
- Required: Current Arkansas RN License.
- Required: Current Arkansas Driver's License.
- PREFERRED: Minimum 1 year of nursing experience.
- PREFERRED: Home Health/Hospice experience preferred.
Qualifications
- This job will be authorized 80.00 hours bi-weekly.

- Knowledge of home health/hospice care practices and regulations.
