Registered Nurse Assessment Coordinator (RNAC)

15 or 22.5 hrs./week
Days - Flexible Hours

Pay

Rate: $43/hr. - $46/hr. (for highly experienced MDS/RNAC may be considered up to $48/hr.)

Responsibilities

• Coordinate and oversee resident assessments and care plans
• Ensure compliance with all regulatory requirements
• Collaborate with interdisciplinary teams to provide comprehensive care
• Open MDS on off-days remotely when necessary
• Participate in ongoing training and development through Relias

Requirements

• Minimum of 1 year of experience as a Registered Nurse
• RNAC specialty required
• Proficiency in Point Click Care for EMR
• Ability to work 2-3 days per week, including Thursdays for care plan meetings
