Registered Nurse Advanced Primary Care

Graduate of an accredited school of nursing. Current RN licensure in Minnesota and Wisconsin (Wisconsin licensure must be obtained within two months of hire). Current BLS certification for health professionals through the American Heart Association or Red Cross (online BLS certification is not acceptable). Strong written and oral communication skills. Preferred: one year of full-time employment as an RN.

Physical Requirements

• Ability to sit or stand for prolonged periods.
• Speech, hearing, and sight skills sufficient to perform the functions of the position.
• Manual dexterity required.
• Proficiency in speaking, reading, and writing English; proficiency in other languages may be required.
• Some positions may have lifting requirements.

Responsibilities

• Center service excellence on patient care and relationships, working effectively as part of a multidisciplinary team.
• Provide comprehensive nursing assessments, evaluate plans of care, and address changes in patient conditions.
• Delegate nursing tasks appropriately, providing supervision and monitoring of functions or activities.
• Create and implement dynamic teaching plans based on identified patient needs.
• Act as a resource for other staff.
• Utilize the nursing process: assessment, problem identification, planning, implementation, and evaluation.
• Demonstrate clinical competence in delivering care to patients and families, implementing established policies, procedures, and standing orders.
• Provide emotional, intellectual, and psychological support to patients and families.
• Document accurately and completely regarding clinical practice.
• Collaborate with the healthcare team, make appropriate referrals, and use interdisciplinary resources to enhance clinical decision-making.
• Ensure continuity of care among home, office, inpatient settings, and community.
• Coordinate care by using health coaching techniques, population management tools, and engaging patients in goal-setting and education.
• Identify and secure patient services, support informed decision-making, and help patients navigate the healthcare system.
• Educate patients, families, and the community on prevention, self-care management, and rehabilitation.
• Participate in patient care conferences, in-services, and educational programs to stay current on best practices.
• Demonstrate leadership through role modeling, decision-making, delegation, communication, and problem-solving.
• Formulate and attain personal and professional goals that align with organizational vision and values.
• Act as a patient advocate, maintain confidentiality, and provide health education.
• Engage in professional growth through conferences, workshops, and continuing education.
• Participate in research to improve patient care and utilize resources efficiently.
• Adhere to nursing standards of conduct, performance, and ethics.
• Contribute to teamwork by resolving problems constructively, sharing knowledge, and placing team goals above individual interests.
• Perform other tasks as assigned by supervisor.
