About the role
Lead the execution and continuous monitoring of individualized patient care plans to ensure optimal recovery progress and high-quality outcomes. Document the patient care journey with meticulous detail, recording conditions and communicating critical updates to the interdisciplinary team for timely intervention. Coordinate all clinical and therapeutic disciplines to ensure a seamless, cohesive, and patient-centered experience from admission through discharge. Execute physician and nursing orders promptly and precisely to maintain the highest standard of patient safety and treatment adherence. Assess patient discharge needs early and collaborate with the care team to develop comprehensive transition plans for a safe and successful continuum of care.

Requirements
- Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
Preferences
- Behavioral health experience preferred.

Qualifications
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
