Registered Nurse, Admissions Home Health

BAYADA Home Health Care is seeking a Full Time Admissions Start of Care Registered Nurse (RN) to join our home health visit team. You will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to remain at home. Oasis experience is preferred.

About the role

Use strong head-to-toe assessment skills with the ability to identify subtle changes in patient condition. Utilize your experience with acute and post-acute care (e.g., CHF, COPD, wound care, IV therapy, post-surgical care). Triage and manage urgent clinical situations independently in the home setting. Perform medication reconciliation, teaching, and safety monitoring. Perform comprehensive wound assessment, care, and documentation.

• Apply strong problem-solving skills in unpredictable home environments
• Manage a dynamic visit schedule and prioritize urgent visits
• Efficiently use point-of-care documentation and technology
• Maintain strong attention to detail while meeting productivity expectations
• Provide care in varied home environments
• Adapt to changing patient needs and schedules
• Demonstrate cultural sensitivity and respect for patient autonomy

Qualifications

• A current nursing license in good standing in the state of Pennsylvania
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
• At least one year of current or recent hands-on medical experience within the past two years with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med surg, sub-acute care or rehab units (medical or skilled nursing settings)

Pay

Weekly Base Pay, depending on qualifications and subject to negotiations: $55-$65 per point.
