About the role

Brief Description Mountain Park Health Center is looking for Registered Dietitians (RD) to join our well established and accredited team of providers. As a large Federally Qualified Health Centers (FQHC) in Arizona, we are dedicated to sustain and improve health by providing excellent, patient-centered primary care.

Responsibilities

• Obtain dietary histories of patients, evaluate their nutritional needs, and provide appropriate medical nutrition therapy collaboratively.
• Create and deliver nutrition education programs for patients.
• Participate and complete clinical quality reviews as needed.
• Help patients understand the relationship between nutrition, food choices and their health condition and assist patients in making appropriate changes to their diet.
• Complete clinical notes in an accurate and timely manner.
• Perform miscellaneous job-related duties as requested.
• Maintains regular and predictable attendance.
• Performs other duties as assigned.

Qualifications

• Minimum Qualifications: Registered Dietitian (RD) certification issued by the Commission on Dietetic Registration (CDR).
• Valid driver’s license.
• Current CPR Certification.
• Preferred Qualifications: Bilingual in English and Spanish.
