Join our team as a Registered Dietitian and make a meaningful impact on seniors' lives. Role Overview: Assess the nutritional status of participants, develop and implement individualized nutrition care plans, and collaborate with the interdisciplinary team to ensure participants' dietary needs are met. Provide counseling, education, and therapy to participants and their caregivers, incorporating cultural and ethnic preferences. Conduct assessments, monitor health indicators such as weight and hydration, and adjust care plans accordingly. Key Responsibilities Conduct comprehensive nutritional assessments and develop personalized care plans.Provide nutritional counseling and education to participants, families, and caregivers.Coordinate nutrition-related interventions like meal planning and supplements.Monitor health changes and communicate findings to the interdisciplinary team with recommendations.Participate in team meetings, quality improvement activities, and training programs.Ensure compliance with safety, privacy, and confidentiality standards.Assist with other duties related to nutritional and participant care as needed. Qualifications & Skills Bachelor’s degree in food, nutrition, or dietetics and passing the Commission on Dietetic Registration (CDR) exam.Minimum of 1 year experience working with elderly populations; clinical nutrition experience preferred.Serve Safe Certification preferred.Excellent communication, teamwork, and cultural competency skills.Valid driver’s license, reliable transportation, and up-to-date immunizations required. Additional Requirements: Successful background check and compliance with safety protocols. The role involves direct participant contact and participation in quality improvement initiatives.
