What You'll Do
- Nutrition assessments and patient care planning.
- Monitor and manage patients requiring tube feeding, dialysis nutrition, wound care, weight management, and other high-risk nutrition interventions.
- Educate patients regarding nutrition to promote wellness.
- Collaborate with RDs, Physicians, Speech & Occupational Therapists.
- Communicate significant nutrition concerns effectively to the interdisciplinary care team to support positive patient outcomes.
- Help improve patient outcomes using “Food as Medicine”.
