Registered Dietitian (RD/RDN)

About the role

Stellar Health Group is seeking a qualified and compassionate Registered Dietitian (RD/RDN) to join our team in Westborough, MA. The ideal candidate will provide clinical nutrition services, develop individualized nutrition care plans, and collaborate with an interdisciplinary team to support resident health and well-being. Candidates with experience in skilled nursing, rehabilitation, or long-term care are encouraged to apply.

Responsibilities

• Conduct nutritional assessments and evaluate residents' nutritional needs.
• Develop and implement individualized nutrition care plans.
• Monitor weight changes, dietary intake, and overall nutritional status.
• Recommend therapeutic diets and nutritional supplements as appropriate.
• Collaborate with nursing, medical, dietary, and other clinical team members.
• Document assessments, interventions, and care plans accurately and timely.
• Participate in interdisciplinary care plan meetings.
• Provide nutrition education and support to residents, families, and staff.
• Maintain compliance with applicable healthcare regulations and facility policies.

Qualifications

• Bachelor's degree in Nutrition, Dietetics, or a related field.
• Current Registered Dietitian (RD/RDN) credential required.
• Must meet applicable Massachusetts licensure requirements.
• Clinical experience in skilled nursing, rehabilitation, or long-term care preferred.
• Strong communication, organization, and clinical assessment skills.
• Ability to work collaboratively as part of an interdisciplinary team.
