Registered Dietitian Per Visit

Responsibilities

• Develops and implements a nutritional plan based on an assessment of nutritional needs of the patient, input from the patient and/or patient representative/caregiver, and current evidence-based practices.
• Includes the plan in the patient's treatment plan and medical record and communicates it to the physician and home health care team.
• Instructs home care staff to:
• Provide appropriate counseling to patients/caregivers in the home setting;
• Evaluate and instruct on nutritional principles, dietary plans, and food selections;
• Adapt plans to the patient's lifestyle as appropriate;
• Re-evaluate nutritional care by providing home follow-up care.

Requirements

• Formal Education and Job-Related Experience: Minimum formal education of a master’s degree in nutrition or dietetics and minimum of six months’ job-related experience.
• Licenses: State of CT Registered Dietitian/Nutritionist license and current valid driver's license.
• Skills: Proficient use of computer-based electronic medical records; ability to calculate figures and amounts, such as proportions, percentages, volume; ability to read, analyze, and interpret clinical documentation from members of the home health team.

Qualifications

• Access to a good working vehicle to support visits to patients' homes.
• Working Conditions: Significant manual skills/motor coordination and finger dexterity; significant occupational risk; significant exposure to dirt, odors, noise, human waste, etc.
