About the role
The Pavilion Rehabilitation and Nursing Center is a Five Star, Joint Commission, American Healthcare Association Bronze Medal Award-winning, short-term rehab, long term, and skilled nursing care community. Conveniently located in Hyannis, we are dedicated to our residents and team members.
Responsibilities
- Completes thorough and accurate initial, quarterly, interim, and annual nutrition assessments for all patients within the time frames established by state and federal guidelines and center policies.
- Completes MDSs and CAAs according to the RAI requirements.
- Develops and reviews nutritional care plans and maintains current, individualized information.
- Identifies residents at nutritional risk and provides assessments and recommendations for nutritional care interventions.
- Collaborates with center team members, physicians, and nurse practitioners to develop nutritional plans of care for residents.
- Analyzes resident weight data trends and reports weight variance concerns to the Director of Nursing.
- Calculates, reports, and analyzes calorie count data as needed.
- Oversees compliance with tube feeding TPN, and PPN documentation and administration.
- Makes sure I&O data is monitored and reported to the Director of Nursing.
- Conducts patient and family education related to nutrition interventions and therapeutic diets.
- Reviews meal production and meal service to monitor menu compliance, portion control, quality and appearance of food, sanitation, operational efficiency, and adequacy of temperatures.
- Performs meal service rounds and sanitation inspections at least monthly or in accordance with centers Performance Improvement Program.
Requirements
- Registered Dietitian License and active member in the American Dietetic Association.
- Three years’ experience in geriatric, administrative, or clinical nutrition.
- Proficient computer skills.
- Ability to perform duties with or without reasonable accommodations.

Schedule
Full-time, 40 hours per week.
