Job Responsibilities include: Perform nutritional assessments and ongoing evaluation of the plan of care for each residentParticipate in IDCP care conferences as neededProvide nutrition education and counseling to residents, families, and staff as needed. Collaborate with interdisciplinary teams to improve residents' overall health and well-beingDemonstrates ability to work in a cooperative manner as a team memberCompletes other duties as assigned.
