Register Nurse Care Manager - Middlesex /Worcester County - Hybrid

For our client, we are seeking a Register Nurse Care Manager - Middlesex /Worcester County - Hybrid to join the team of a leader in the Healthcare space. This role will lead technical initiatives focused on scalable systems, platform reliability, and meaningful business transformation. You will work across engineering, product, operations, and business stakeholders to translate complex requirements into practical technology solutions. The position offers the opportunity to influence architecture, execution quality, and the technology capabilities that enable long-term growth within a healthcare environment. Location: Remote - US based candidates only, no visa sponsorship available Compensation: $95,000 – $105,000 annually Responsibilities Receive and communicate program expectations clearly to members Perform assessments using specific clinical screeners to identify needs Conduct in-person clinical examinations as needed, collaborating for appropriate member placement Participate in case conferences, leading discussions when necessary Develop and implement care plans in collaboration with members, addressing social needs Monitor routine interventions through regular clinical visits and follow-ups Support the team in managing chronic diseases and behavioral health issues Qualifications Graduate of an accredited nursing program (R.N.) 3+ years of nursing experience, preferably in care management Strong critical thinker with the ability to make independent decisions Excellent written and verbal communication skills, comfortable using technology Proven experience in applying Motivational Interviewing and Trauma-Informed Care principles Benefits Health insurance Life insurance Retirement benefits Participation in the company's equity program Paid time off, including vacation and sick leave Our client is an equal opportunity employer.
