Regional Route-Home 2 days 6-10

About the role

A regional route-home position is available for a 2-day per week schedule from 6-10 PM.

Responsibilities

• Provide home visits to clients in the community
• Assist clients with accessing necessary resources and services
• Document client interactions and progress in a timely manner
• Participate in regular team meetings to discuss case management strategies

Requirements

• Valid driver’s license and reliable transportation
• Ability to work independently and manage time effectively
• Strong interpersonal and communication skills
• Proficient in Microsoft Office Suite

Qualifications

• Bachelor’s degree in Social Work, Public Health, or related field
• At least 2 years of relevant experience in social service or healthcare field
• Knowledge of local community resources and services
