REGIONAL HOME CARE PHYSICAL THERAPIST

Job Responsibilities

• Direct Participant Care
• Perform participant evaluations and treatment
• Document all pertinent information as required
• Develop a patient care plan with interdisciplinary team members
• Maintain a clean, safe environment and follow safety protocols
• Maintain the inventory of supplies and equipment
• Provide clinical instruction, mentoring, and direction to PTA’s and students
• Compliance and Quality Assurance
• Audit and review patient records, documentation, and progress reports
• Maintain compliance with state and federal regulations, accreditation standards, and company policies
• Participate in quality improvement initiatives and audits
• Administrative Responsibilities
• Maintain up-to-date records and documentation
• Schedule and coordinate therapy sessions
• Attend administrative and interdisciplinary team meetings
• Provide reports on team performance, patient outcomes, and operational needs

Qualifications

• Master's or Doctoral degree in Physical Therapy from an accredited program
• Current, valid New York State Physical Therapy license
• Two (2) years of experience preferred
• Experience in clinical practice across a range of settings preferred
• Geriatric experience is preferred
• Knowledge of PACE regulations
• Knowledge of Care Compass and other EMRs preferred
• Bilingual preferred depending on region (participant demographics and location of PACE sites)

Physical Requirements

• Standing – Duration of up to 6 hours a day
• Sitting/Stationary positions – Sedentary position in duration of up to 6-8 hours a day for consecutive hours/periods
• Lifting/Push/Pull – Up to 50 pounds of equipment
• Bending/Squatting – Must be able to safely bend or squat to care for patients
• Stairs/Steps/Walking/Climbing – Must be able to safely maneuver stairs, climb up/down, and walk to access work areas
• Agility/Fine Motor Skills – Must demonstrate agility and fine motor skills to operate and activate equipment, devices, instruments, and tools
• Sight/Visual Requirements – Must be able to visually assess patients, read orders type/write documentation, etc. with accuracy
• Audi/Hearing and Motor Skills (language) Requirements – Must be able to listen attentively and document information from patients, community members, providers, etc., and intake information through audio processing with accuracy
• Cognitive Ability – Must be able to demonstrate good decision-making, reasonableness, cognitive ability, rational processing, and analysis to satisfy essential functions of the job

Salary Range

$123,000.00 - $126,000.00
