About the role
Provides hospice care services to patients and families in their homes. Works closely with healthcare providers to coordinate care plans.
Responsibilities
- Assesses patient needs and develops personalized care plans
- Administers medications and treatments as prescribed
- Makes rounds to patients to monitor progress and adjust care plans as necessary
- Communicates with family members and other healthcare providers to ensure coordinated care
- Completes documentation and reports as required
Requirements
- Bachelor's degree in a healthcare-related field
- Current CPR certification
- Experience in hospice care or related field preferred
Qualifications
- Strong communication skills
- Ability to work independently and as part of a team
- Flexibility to work evenings and weekends
Skills
- Excellent interpersonal skills
- Knowledge of hospice care protocols and procedures
- Proficient in using electronic health records (EHR)
