Refugee Health Coordinator, Limited Term - 2025837

About the Role

This position will assist refugees and immigrants in obtaining their Initial Health Screening, establishing a primary care provider, and providing follow-up care. You will offer health education and orientation to the U.S. healthcare system through sessions with newly arrived refugees and additional workshops. This role involves close collaboration with transportation volunteers and case managers to coordinate ongoing health needs for clients, as well as providing other refugee case management support as needed.

This is a limited-term position funded through a grant agreement until 6/30/2027 and contingent upon funding extension. Hybrid schedule: 4 days in office, 1 day remote per week.

Responsibilities

• Schedule health screening appointments for refugee clients within 30-90 days of arrival
• Assist with transportation planning and provision, and connect clients to public transportation
• Ensure follow-up immunizations are scheduled and barriers to care are addressed with the client
• Maintain good communication with the health screening clinic and attend bi-monthly meetings to discuss collaboration and client coordination
• Coordinate with the health screening clinic to obtain referrals for follow-up appointments
• Provide orientation on the U.S. healthcare system to new arrivals, including proper filling and following of prescription instructions, medical insurance, and when to use primary care vs. the emergency room
• Provide referrals to community resources and other service providers, including assisting clients without health insurance in accessing free or low-cost healthcare when possible
• Coordinate with medical providers to equip clients to follow their plan of care as directed
• Assist clients in identifying a local primary care provider if necessary
• Ensure clients have access to a local pharmacy and assist with initial filling of prescriptions
• Promote the health of immigrants through coordination of health workshops and collaboration with local healthcare providers
• Work closely with internal department teams and external service providers to ensure comprehensive service provision, including other Resettlement team members
• Assist clients with applying for, completing redeterminations for, and navigating health insurance and other public benefits
• Help clients obtain important documents, IDs, and other necessary paperwork
• Conduct regular client follow-ups as needed to check on well-being, track client needs, and follow up on them until resolved
• Assist clients in applying for heat and energy assistance through LIHEAP (Low-Income Home Energy Assistance Program)
• Take ownership of assigned cases or specific case management tasks when needed
• Assist with program closures and required documentation
• Maintain timely and accurate client files, case notes, and reporting records
• Participate in case staffing and discuss client progress and barriers
• Actively participate in department meetings, staff development opportunities, program trainings, community activities, and organizational events
• Collaborate with volunteers and Volunteer Services staff to coordinate the work of volunteers engaging with clients
• Other duties as assigned

• Able to affirm and/or acknowledge World Relief’s Core Beliefs, Statement of Faith, Christian Identity, and National Association of Evangelicals' For the Health of The Nation document
• Valid driver’s license, access to reliable vehicle, and ability to pass MVR standards
• MS Office skills required
• Ability/willingness to drive a 15-passenger van
• Ability to work occasional early mornings, evenings, or weekends as necessary

Preferred Qualifications

• Demonstrated compassion, patience, cross-cultural sensitivity, and strong mediation skills
• Ability to navigate the American healthcare system, including Medicaid
• Adaptable and possessing crisis management skills
• Able to communicate well with limited English-speaking clients and with healthcare professionals
• Demonstrated teaching and coaching skills with the ability to provide ongoing orientation to refugee participants
• Strong organizational skills and attention to detail
• Highly self-motivated with strong follow-through and ability to work independently
• Fluency in an additional language such as Dari, Pashto, Arabic, Swahili, French, or Spanish strongly preferred
