REACH Care Manager (RN)

About the role

REACH Kidney Care, a division of Dialysis Clinic, Inc., is a kidney health management program that supports patients from early stages of kidney disease through advanced stages, including those with end-stage renal disease (ESRD).

Responsibilities

• Educate patients and families on CKD management and modality choices.
• Carefully coordinate services with interdisciplinary team members (social worker, dietitian, pharmacist).
• Review medical records and update REACH EMR.

Qualifications

• Graduation from an accredited nursing program (BSN, AA, diploma).
• Prior experience as a registered nurse preferred, with 1 year of dialysis experience required.
• Current Registered Nurse license.
• Must be 21+ with a valid driver's license and acceptable driving history.

Pay

Pay range from $83,200 annually, depending on nursing and dialysis experience.
