About the role
The primary responsibility is to assemble, monitor, maintain, retrieve, and file patient information in the electronic health record according to HIM policies and procedures. Duties also include Release of information, HIPAA compliance, handling Warrants/Subpoenas, Guardianship, and Designation of Personal Representatives, including electronic submission of birth certificates to the State of New Hampshire, Vital Records.
Responsibilities
- Maintains the master patient index (MPI) integrity within the current and old systems
- Takes corrective actions for any discrepancies in reports
- Notifies HIM Manager, Privacy Officer, and Risk Management of potential liability issues in documentation
- Gathers, organizes, reconciles, manages, scans, and uploads patient records to the EMR
- Conducts quality checks and completes/analyzes deficiencies for all services provided within the facility
- Reports deficiencies to appropriate staff and monitors for completion
- Responds to Patient Portal queries, such as enrollment requests or assistance with password resets
- Accurately submits electronic birth certifications using the State of New Hampshire Vital Records prescribed software
- Maintains advanced direct avh staff push x-ray images to DHMC electronically and burns images to CD as requested
- Retrieves and distributes patient records for professional practices
- Receives new patient packets, logs them, notifies appropriate departments, scans documents per policy
- Maintains a clean and orderly work area
- Monitors the fax inbox
- Handles HIPAA compliant, Warrants/Subpoenas, Guardianship, and Designation of Personal Representative requests
- Maintains cooperative relationships with colleagues, handling difficult situations consistently and fairly
Requirements
- Education: HS Diploma, GED, or successful completion of an accredited HIM program through AHIMA or AAPC with certification
- Licensure: Registration and Certification
