Responsibilities
- Develops professional and clinical skills through attendance at staff development meetings, pertinent educational activities, and professional affiliations.
- Participates in the review process and the agency performance improvement program.
- Provides patient education, medication administration, wound dressing changes, and ostomy care as directed by the RN and ordered by the physician.
- Displays flexibility in accepting assignment changes and participates in weekend/after-hours coverage and call rotation.
- Reports pertinent observations to the attending physician and RN, and implements changes in the patient care plan as directed.
- Communicates changes to the appropriate members of the home health care team.
- Demonstrates effective communication skills with all team members, patients, and families.
- Maintains clinical record information in accordance with established standards and within established time frames.
- Provides direct patient care responsibilities for assigned patients in an efficient, organized manner under the direction of the RN.
- Performs treatments within their scope of training and administers medications to assigned patients.
- Assesses patients during clinical check-in, documents pertinent medical information and vital signs, and assesses for signs of abuse or neglect using age-specific guidelines.
- Administers treatments, medications, and follow-up assessments ordered by the provider, verifying as needed, and responds appropriately in clinical emergencies and triage situations.
- Performs diagnostic and occupational medicine testing, including EKG, spirometry, audiometry, titmus, and pulse oximetry, and complete forensic testing collections in accordance with established guidelines.
- Collects data specific to the age of the patient and contributes to their plan of care.
- Receives, verifies, and implements physician orders, ensuring orders taken by the LPN are co-signed by an RN.

- Demonstrates knowledge of practical nursing theory, modalities, treatments, and skills.
- A high level of multitasking, organization, and proactive communication skills are necessary.

- Basic computer skills required including but not limited to MS Office
- Additional computer knowledge [Preferred]
- Knowledge with a Point of Care software system for patient care documentation [Preferred]
Qualifications
- High School Grad or Equiv [Required]
- Technical/Vocational School graduate of an accredited school of licensed practical nursing [Required]
- Field Of Study Nursing Technical/Vocational School - Graduate of an Accredited School of Licensed Practical Nursing Required
