About the role
Identify specific performance improvement initiatives and work collaboratively with local agencies/leaders in their development, implementation and evaluation of quality improvement programs.
Collaboratively monitor the implementation of identified processes to assure continued agency licensure, accreditation and coverage compliance requirements.
Perform workflow review and completion to achieve standardization and organizational efficiencies.
Work collaboratively with agency to ensure compliance with new regulations and standards in EMR documentation.
Responsibilities
- Identify and collaborate on the development of performance improvement initiatives
- Monitor and evaluate the implementation of quality improvement programs
- Maintain compliance with licensure, accreditation, and coverage requirements
- Review and complete workflows to enhance standardization and efficiency
- Ensure compliance with new regulations and standards in EMR documentation
Requirements
- Registered Nurse/Therapist with current license from respective State Board
- Bachelor’s degree in nursing or therapy (Physical, Occupational, or Speech)
- OASIS Certification (COS-C or HCS-O) required within 6 months from date of hire
- Maintain recertification with no lapses in certification period
- Four years' experience in home care
- Minimum two years in a supervisory, oversight role
- Experience serving on committees or other leadership activities
- Develop interpersonal and communication skills to provide effective interface with agency leaders, professional clinical staff and all other related team members
- Valid Driver’s License and reliable transportation to and from the work site
