About the role
The W Group is seeking a Quality Auditor to join their compliance team. This is a field-based, multi-facility role located in Brooklyn, NY, with some travel required across all W Group facilities.
Responsibilities
- Congduct scheduled internal compliance audits across all W Group facilities using standardized tools and checklists developed by the Compliance Officer
- Gather objective evidence through documentation review, direct observation, and structured staff interviews
- Complete thorough audit reports documenting findings and observations; submit to the Compliance Officer prior to any communication to facility leadership
- Conduct re-audits to assess the effectiveness of corrective actions
- Verify that Plans of Correction are being carried out as written
- Flag corrective actions that are incomplete, ineffective, or not yet implemented
- Maintain organized audit records in our centralized tracking system (ALIS)
- Identify recurring findings and cross-facility patterns for the Compliance Officer
- Assist in compiling audit data for compliance trend reports
- Support DOH survey readiness activities at facilities as directed
- Identify documentation gaps and operational vulnerabilities ahead of surveys
- Provide on-site support during active DOH surveys as needed
- Build respectful, professional working relationships with facility administrators and staff
- Communicate audit scope and purpose clearly at each site visit
- Refer all regulatory interpretation questions to the Compliance Officer
Requirements
- Bachelor's degree required; Healthcare Administration, Nursing, Social Work, or related field preferred
- 1-3 years of experience in assisted living, ACF, ALP, or LHCSA healthcare compliance operations
- Familiarity with NYS DOH survey processes and ACF/ALP/LHCSA regulatory requirements
- Prior experience conducting audits, inspections, or structured compliance reviews preferred
- Strong written communication and documentation skills
- Highly organized and self-directed -- comfortable managing an independent field schedule
- Proficiency with electronic records systems; ALIS experience a plus
- Valid driver's license and reliable transportation required
