Public Health Specialist I - Chronic Disease

About the role

The Vermont Department of Health Rutland District office seeks a Public Health Specialist I - Chronic Disease to lead systems-level approaches for improved access to physical activity, nutrition, tobacco prevention, oral health, and preventive care for underserved communities. The ideal candidate understands health inequities and uses public health frameworks with community-based coalitions.

Responsibilities

• Provide leadership to communities using data to assess, prioritize, and develop sustainable interventions aimed at improving chronic disease outcomes.
• Work collaboratively to develop the capacity of the community to improve access to physical activity, nutrition, tobacco prevention, oral health, and preventive care.
• Evaluate the process, short-term and long-term outcomes of prevention interventions.
• Provide technical support for the integration of health equity and access to care strategies within municipal and organizational planning.
• Participate in and support community-based coalitions that examine health data, set goals, and develop plans to improve health outcomes.
• Enlist civic and municipal leaders as well as leaders at the grassroots level to help advance local community policy and program agendas.
• Prioritize and make decisions based on community input and local data tools including Community Health Needs Assessment, YBRS, and Department of Health data.

Requirements

• Two (2) years or more of experience in public health, human services, emergency response, community engagement, program administration, human services related fields or health-related fields.
• OR Associate degree in public health, public administration, business administration, environmental science, emergency response, human services, social sciences related fields or health-related fields and one (1) year or more of experience in public health, human services, emergency response, community engagement, program administration, human services related fields or health-related fields.
• OR Bachelor’s degree in public health, public administration, business administration, environmental science, emergency response, human services, social sciences related fields or health-related fields.

Qualifications

• Experience in public health, human services, emergency response, community engagement, program administration, human services related fields or health-related fields.
• Associate degree in relevant field with one (1) year of experience.
• Bachelor’s degree in relevant field.

Skills

• Understanding of root causes of health inequities.
• Experience using public health frameworks with community-based coalitions.
• Extensive relationship building with staff, state partners, and community partners.
• Ability to embrace unique perspectives and foster an inclusive environment.
