Public Health Registered Nurse, Level 1, 2, or 3 (based on qualifications/experience).

About the role

Provides home visiting services within the 14-county health district to identify, assess, and provide care coordination services to eligible perinatal patients and qualifying infants. Identifies high-risk populations and tailors interventions based on the needs of the population and resources available. Promotes the program through community engagement and outreach events. Provides referrals and linkages to appropriate services when medical and environmental risks are identified. Responds to public health emergencies.

Responsibilities

• Provides professional nursing care in accordance with nurse protocols, policies, and/or procedures under broad supervision.
• Maintains outstanding customer service and HIPAA standards related to privacy and security.
• Must maintain CPR certification.
• May require driving company vehicles (a valid driver’s license is necessary) and working some evenings and weekends during special events such as health fairs and disaster responses.

Requirements

• Associate degree in nursing from an accredited college or university.
• Completion of a PHN course within one (1) year of hire.
• Current Georgia Registered Professional Nurse license and CPR certification.
• Occasional lifting of up to 45 lbs. and the ability to bend and reach up.
• Consent to a motor vehicle record assessment (points should be fewer than 10 and should not negatively impact insurance rates for the agency).
• Drug screening, criminal background check, and immunization records may be required.
• If selected, male applicants aged 18 to 26 must provide proof of Selective Service registration.

Preferred Qualifications

• Bachelor’s or Master’s degree in nursing, public health, or related field from an accredited college or university.
• Previous employment with any state’s Department of Public Health or a County Board of Health.
• Experience conducting home visits and patient home assessments.
• Experience with providing intervention services for high-risk newborns and infants.
• Experience with case management and community outreach.
• One or more years of labor and delivery experience.
• Bilingual (Spanish and English).

Pay

• Level 1 (Job Code HCP190): $51,811.26
• Level 2 (Job Code HCP191): $58,547.06
• Level 3 (Job Code HCP192): $66,168.17
