Public Health Registered Nurse: Immunization and Communicable Disease

Essential Functions/Responsibilities

  • Conduct health assessments, gather histories, administer diagnostic tests, and provide nursing services such as vitals checks, immunizations, and medication administration.
  • Build, maintain, conduct processes and outcome measures of supportive grant programs, adhering to grant reporting guidelines as assigned.
  • Develop individualized care plans based on client needs and goals, implement and re-evaluate these plans, and make referrals to other providers or resources as needed.
  • Manage vaccines, oversee off-site clinics, educate clients on immunizations, and maintain records in the state immunization system.
  • Conduct TB and communicable disease screenings, provide education, manage treatment plans, and perform epidemiological investigations in line with CDC and CDPHE guidelines.
  • Support sexual and reproductive health services, chart, check in patients, collect and label lab samples, and educate.
  • Perform care coordination, develop family care plans, and manage documentation for child health programs.
  • Collaborate with team members to support overall clinic operations, ensuring efficient and effective care across all service areas.
  • Offer counseling, education, and outreach to individuals, families, and communities on disease prevention, wellness, and available health programs.
  • Maintain accurate records in compliance with health department policies, including using electronic health records and following HIPAA standards.
  • Demonstrate competencies in the Incident Command System for public health emergencies (NIMS-700, ICS-100, ICS-200).

Requirements

  • Associate’s degree in nursing required; bachelor’s degree preferred.
  • At least three (3) years of nursing experience in a Public Health setting, physician’s office, or community clinical setting preferred.
  • Proficiency and experience in AthenaOne (electronic health record) is preferred.
  • Current and unrestricted Colorado Registered Nurse license.
  • Current and unrestricted American Heart Association BLS certificate.
  • Current and valid Colorado Driver’s License.

Physical Requirements

  • Walk, stand, sit, type, use hands and fingers, handle or feel objects, reach with hands and arms, talk, listen, see, grasp, pull, lift, drive, stoop and bend to examine patients.
  • Climb in/out of vehicle, physically restrain children with approved techniques, transport equipment and supplies.
  • Lift: 50-pound requirement.
  • Exposure to Elements: Normal office conditions, position requires travel to other locations in the community and state-wide.
  • Handling of Dangerous Equipment: Syringes, needles, medical examination equipment, emergency response equipment.
  • Risk of Assault or Injury: May receive verbal assault from angry citizens, possible contaminated samples, bio hazardous material, body fluids, latex, alcohol, communicable diseases, germicide, decontamination chemical, air and car travel, possible physical assault from children.
