About the role

We are seeking a Public Health Nurse to provide comprehensive client care, including health assessments, developing and coordinating care plans, and conducting office and home visits during pregnancy and postpartum periods.

Responsibilities

• Evaluating referrals
• Collaborating with healthcare teams
• Maintaining client contact
• Offering health education on prenatal, postpartum, and newborn care
• Mentoring staff
• Ensuring compliance with regulatory standards

Qualifications

• Bachelor's Degree in Nursing or equivalent experience
• RN licensure
• BLS certification
• Preferably bilingual skills (English/Spanish)
• Knowledge of nursing procedures, therapeutic communication, and EMR systems
