About the Role
CareMed Health Solutions is a facility-based team of medical providers delivering medical services to patients in long-term care facilities. As a Wound Care Nurse Practitioner, you will provide wound care services to residents, impacting their lives and well-being positively while collaborating with an interdisciplinary care team.
Responsibilities
- Provide wound care services for long-term care patients residing in skilled nursing facilities.
- Prescribe appropriate medications, including controlled substances in accordance with Ohio law.
- Order, review, and/or follow up with treatments and diagnostic testing.
- Communicate regularly with nursing home staff regarding changes in patient condition and care plans.
- Collaborate with the primary care physician.
Requirements
- Current, unrestricted Nurse Practitioner license in the state of Ohio.
- Board Certification.
- DEA licensure (optional).
- Clinical background in adult, family, or geriatric specialties.
- Experience in wound care, including chronic, debilitating, stalled, or infected wounds.
- Comfort diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
- Ability to stage pressure injuries according to the NPIAP.
- Familiarity with wound care best practices and advanced wound care products (e.g., collagens, alginates, foams, silvers).
- Experience with wound vacs and other durable medical equipment (DME), such as pressure-relieving/reducing devices.
- Strong preference for wound certification or willingness to obtain certification.
- Experience working in a long-term care setting.
- Ability to work with an EMR.
- Ability to work within an interdisciplinary care team to coordinate patient care.
