About the role

Help residents move toward their next recovery milestone. Stern Therapy Consultants is seeking a PRN Physical Therapist at Autumn Lake at Salem County, providing skilled nursing rehabilitation that connects clinical treatment with residents' everyday mobility needs.

Responsibilities

• Evaluate mobility, strength, balance, and range of motion while considering medical precautions and prior function.
• Develop individualized plans of care with measurable goals for walking, transfers, and daily movement.
• Provide therapeutic exercise, gait training, and functional mobility treatment appropriate to each resident.
• Assess assistive device needs and educate residents and caregivers on safe movement and fall prevention.
• Reassess progress, adjust interventions, and supervise PTA services within the treatment plan.
• Document clinical findings and outcomes, coordinating recommendations with nursing and the rehabilitation team for ongoing care and discharge.

Qualifications

• Active New Jersey Physical Therapist license.
• Strong evaluation skills and sound clinical judgment.
