About the Role

We are seeking a Physical Therapist (PT) for a PRN position at Autumn Lake at Chesapeake Woods in Cambridge, MD. Join a supportive rehabilitation setting where you can provide individualized therapy focused on improving residents' strength, mobility, balance, and functional independence.

Responsibilities

• Perform physical therapy evaluations and establish individualized plans of care
• Provide skilled interventions for strength, balance, gait, transfers, endurance, and mobility
• Establish functional goals and track resident progress
• Adjust treatment plans according to clinical needs and response to therapy
• Educate residents and caregivers on exercises and safe mobility techniques
• Maintain clear and timely clinical documentation
• Coordinate care with PTAs and other members of the interdisciplinary team

Qualifications

• Active Maryland Physical Therapist license required
• Graduate of an accredited Physical Therapy program
• Strong clinical, communication, and teamwork skills
• Skilled nursing or post-acute experience preferred, but not required
• New graduate Physical Therapists are welcome to apply

$55–$60/hour
