About the role

Civita Care Center at Long Ridge is seeking a PRN Physical Therapist (PT) to join our rehabilitation team. This flexible opportunity is ideal for a clinician who enjoys working in a collaborative skilled nursing environment while helping residents improve mobility, restore function, and achieve greater independence.

Responsibilities

• Perform physical therapy evaluations and develop individualized treatment plans
• Provide evidence-based interventions to improve strength, balance, mobility, endurance, and functional performance
• Monitor resident progress and adjust treatment plans as appropriate
• Collaborate with Physical Therapist Assistants, Occupational Therapists, Speech-Language Pathologists, nursing staff, and physicians
• Complete timely and accurate documentation in accordance with facility and regulatory standards

Qualifications

• Active Connecticut Physical Therapist license required
• Strong clinical assessment and communication skills
• Skilled nursing or rehabilitation experience preferred, but not required
