What You'll Do
- Perform physical therapy evaluations and establish individualized plans of care
- Provide skilled interventions focused on mobility, strength, balance, gait, and functional performance
- Monitor resident outcomes and modify treatment plans when appropriate
- Educate residents and caregivers on mobility and safety strategies
- Work collaboratively with PTAs, nursing, and other rehabilitation professionals
- Complete required clinical documentation
Qualifications
- Active Massachusetts Physical Therapist license required
- Strong clinical assessment and communication skills
- Skilled nursing or post-acute care experience preferred
