About the role

Flagstar Rehab is seeking Physical Therapists for Rehab Department positions in state-of-the-art skilled nursing facilities throughout NYC and Surrounding Areas. Flagstar Rehab PTs work with a diverse and multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow
• Supervise PTAs daily
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson's, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis
• Monitor and document patient progress with the use of innovative EMR software

Pay

$46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Requirements

• Current NYS PT License and Registration
• CPR Certification
• EMR Competence
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)
