PT Nurse Practitioner - Phoenix
About the role
Pine Park Health is looking for a Part-Time Nurse Practitioner responsible for delivering care in our Phoenix, Arizona market (Glendale/Sun City area). If you are excited about the opportunity to work with a smaller patient panel of complex care patients, contribute to a value-based care model, and work with a motivated multi-disciplinary team to provide high-quality patient care, this role is for you. This person-centered care model leverages the power of our care navigators to maximize the time and energy of our providers.
Responsibilities
- Provide onsite primary care services to older adults living in Senior Living Communities (2-3 days per week)
- Develop and execute detailed person-centered care plans for all primary care patients
- Work with the Medical Director to identify and develop policies addressing day-to-day practice, culture, or clinical guidelines, including emerging diseases
- Provide monthly Chronic Care Management (CCM) visits and care; review and sign off on CCM documentation
- Run regular huddles and provide leadership to the clinical team
- Identify ways to collaborate with senior living management and care teams to better carry out Pine Park care plans and improve patient outcomes
Qualifications
- Nurse Practitioner certification (preferably with geriatrics experience as an AGNP)
- Comfortable with a fast-paced and changing environment
- Willingness to provide onsite clinical care across the Glendale/Sun City region
- Desire to work with complex care patients and support the building of a holistic care model
- Strong orientation toward collaboration and working closely with a multidisciplinary team; values input from all members of the clinical team
- Strong written and verbal communication skills
- Ability to work with tech-enabled care delivery tools (e.g., project management, checklist management, remote monitoring) to improve patient care and team function
- Flexible work schedule with travel within the Phoenix area; this role involves driving between partner communities as needed
