PT Days Certified Nursing Assistant (24 hours/week)

About the Role

Under the direction of a Coastal Hospice registered nurse, you will provide personal care, assist with activities of daily living, homemaking, and related support to patients. This role involves delivering direct patient care as instructed by a registered nurse or physical therapist, and rotating assignments to meet the 24/7 needs of the Coastal Hospice program.

Responsibilities

• Provide emotional support to patients and their families, requiring strong interpersonal skills.
• Relieve primary caregivers for occasional respite.
• Deliver 1:1 care and other assigned assistance to patients and families.
• Communicate effectively and promptly with the supervising nurse and other hospice aides by:
  • Documenting services clearly and accurately by the end of each shift.
  • Promptly reporting any significant changes in a patient’s condition.
• Comply with Coastal Hospice policies by:
  • Demonstrating knowledge of and alignment with Coastal Hospice policies and care philosophy.
  • Documenting 12 hours of in-service training annually.
  • Successfully completing periodic competency examinations and performance evaluations.
• When assigned to Coastal Hospice at the Lake, perform these additional duties:
  • Communicate effectively during change-of-shift rounds.
  • Assist with unit operations by:
    • Reporting low levels of linens and supplies.
    • Checking and documenting daily refrigerator temperatures.
    • Maintaining a safe physical environment according to unit needs and regulatory standards.
    • Performing light housekeeping duties.

Qualifications

• Current CNA certification in Maryland.
• Successful completion of training required by hospice regulations and standards.
• Ability to understand and follow spoken and written instructions.
• Sight, hearing, and manual dexterity sufficient to perform job requirements, with or without the use of prosthetics.
• Ability to lift, push, pull, or move patients, equipment, or supplies in excess of 50 pounds, with or without adaptive/assistive devices.
• Ability to navigate level and uneven surfaces, with or without assistive devices.
