Psychotherapist, Palliative Care

Position Overview

We are seeking an Advanced Certified Hospice and Palliative Social Worker (ACHP-SW) to join our team at PAMF-Palo Alto Medical Foundation. This role provides integrative psychotherapy through expressive arts therapy, comprehensive assessments, and counseling support for patients with chronic or life-threatening illnesses.

Responsibilities

• Provides integrative psychotherapy and counseling support for mental, emotional, social, and behavioral issues.
• Affords comprehensive assessments and counseling support for patients with chronic or life-threatening illnesses.
• Assists patients in coping with stress, grief, and loss, and making healthy behavioral changes.
• Works in partnership with physicians, other health practitioners, and administrative staff on care coordination and treatment planning.
• Collaborates with other psychotherapists to implement and sustain best practices in integrative psychotherapy and guided imagery.
• Stays updated on research and best practices in behavioral health and integrative medicine.

Requirements

• Master's Degree in a Mental Health Discipline
• LCSW - Licensed Clinical Social Worker
• LMFT - Licensed Marriage & Family Therapist
• PSY - Psychologist of California
• 2+ Years of Recent Relevant Experience

Skills and Knowledge

• Ability to assess patient needs and provide high-quality integrative psychotherapy.
• Knowledge of holistic assessment, care plan development, and outcomes tracking methods.
• Capacity to work within the integrative medicine model as practiced at the Institute for Health & Healing.
• Desire and ability to work as an active member of an interdisciplinary practice team.

Education

The ideal candidate will have a Master's Degree in a Mental Health Discipline and be licensed as LCSW, LMFT, or PSY in California.
