Position Overview
We are seeking an Advanced Certified Hospice and Palliative Social Worker (ACHP-SW) to join our team at PAMF-Palo Alto Medical Foundation. This role provides integrative psychotherapy through expressive arts therapy, comprehensive assessments, and counseling support for patients with chronic or life-threatening illnesses.
Responsibilities
- Provides integrative psychotherapy and counseling support for mental, emotional, social, and behavioral issues.
- Affords comprehensive assessments and counseling support for patients with chronic or life-threatening illnesses.
- Assists patients in coping with stress, grief, and loss, and making healthy behavioral changes.
- Works in partnership with physicians, other health practitioners, and administrative staff on care coordination and treatment planning.
- Collaborates with other psychotherapists to implement and sustain best practices in integrative psychotherapy and guided imagery.
- Stays updated on research and best practices in behavioral health and integrative medicine.
Requirements
- Master's Degree in a Mental Health Discipline
- LCSW - Licensed Clinical Social Worker
- LMFT - Licensed Marriage & Family Therapist
- PSY - Psychologist of California
- 2+ Years of Recent Relevant Experience
Skills and Knowledge
- Ability to assess patient needs and provide high-quality integrative psychotherapy.
- Knowledge of holistic assessment, care plan development, and outcomes tracking methods.
- Capacity to work within the integrative medicine model as practiced at the Institute for Health & Healing.
- Desire and ability to work as an active member of an interdisciplinary practice team.
Education
The ideal candidate will have a Master's Degree in a Mental Health Discipline and be licensed as LCSW, LMFT, or PSY in California.
