Key Responsibilities

  • Carry out comprehensive mental health assessments and diagnostic evaluations.
  • Provide individual psychotherapy and ongoing treatment services.
  • Develop, implement, and update individualized treatment plans in collaboration with a multidisciplinary care team.
  • Maintain accurate, timely, and compliant clinical documentation.
  • Carefully coordinate care with facility staff, medical providers, and other healthcare professionals.
  • Adhere to all federal, state, and organizational standards, policies, and regulations.

Qualifications

  • Active Kentucky license as one of the following: LCSW, LPCC, LMFT, or Licensed Psychologist – PsyD/Ph.D.
  • Master’s degree (LCSW, LPCC, LMFT) or Doctoral degree (Psychologist) in a relevant mental health field.
  • Ability to work independently while collaborating effectively within a clinical team.
  • Strong communication and clinical documentation skills.
  • Preferred: Experience working with geriatric populations, experience in Skilled Nursing Facilities, Assisted Living, or long-term care settings.

Employment Type

Pay-per-encounter, fee-for-service compensation.
