Provide per diem home and community-based mental health services.

Responsibilities

• Conduct comprehensive psychosocial and psychiatric assessments.
• Develop person-centered care plans tailored to individual needs.
• Deliver therapeutic interventions in collaboration with an interdisciplinary team.
• Coordinate community resources and support families and caregivers.
• Respond to crises to ensure safety and stabilization.

Qualifications & Skills

• Licensed Master Social Worker (LMSW).
• Strong commitment to compassionate, recovery-oriented care.
• Ability to work effectively in dynamic, field-based settings.
• Excellent communication and collaboration skills.
